Risk of Rain 2 items change your Survivor through passive effects, usable equipment, and expansion-specific systems. Read the effect and its trigger before judging a pickup by its color. A Common item that supports the attacks you are using can be more useful to your current run than a rarer item whose condition you rarely meet.
Start by covering four jobs: reaching safe positions, dealing reliable damage, recovering health, and handling groups. Then examine how another copy changes the particular effect. Stacking is item-specific, and a displayed chance or health tradeoff does not always grow through simple addition.
Identify the item category before picking it up
White, green, and red borders identify Common, Uncommon, and Legendary items. Yellow identifies Boss or Planet items. Lunar items use blue, and Void items use purple. Expansion categories add further distinctions, so color alone does not describe every acquisition rule or drawback.
Most passive items appear in the inventory bar and keep affecting you while you hold them. Removing one through an inventory exchange removes its effect. You can hold many types and many copies of passive items, but that does not mean that every additional pickup changes your build in an equally useful way.
Equipment is different: you normally hold one active item and use it for an effect followed by a cooldown. Picking up another equipment item swaps the one you are carrying. A new equipment pickup is therefore a replacement decision, rather than another permanent passive bonus added to the same inventory.
Some items require a challenge unlock before they can appear in your item pool. Finding an item also unlocks its logbook entry. An enabled expansion, a completed challenge, and a collected logbook entry describe different kinds of progress; check which one matters when an item is absent from the choices you expected.
Choose movement that matches the action you need
Paul's Goat Hoof increases movement speed by 14% per stack. Energy Drink increases sprint speed by 25% per stack. The distinction is practical: if you need to move while performing an action that prevents sprinting, a sprint-specific benefit is not the same as an ordinary movement-speed increase.

Hopoo Feather increases maximum jump count by one per stack. It solves a different problem again: access to another jump does not directly increase running speed. Use it when changing height, crossing terrain, or altering an airborne route is what you need.
When choosing between these effects, describe the situation concretely. Are you taking too long to cross open ground, failing to leave an attack while fighting, or unable to reach a useful elevated position? Matching the pickup to the missing action gives a more useful answer than putting every mobility item into one interchangeable category.
Movement also supports damage and recovery indirectly by helping you maintain the conditions those items require. An attack that you cannot safely aim, or a healing window you cannot reach, is not improved merely by adding more raw damage to the inventory.
Separate attack speed, critical hits and conditional damage
Soldier's Syringe increases attack speed by 15% per stack. Lens-Maker's Glasses adds a 10% critical-strike chance per stack, with critical hits dealing double damage. Those effects change different parts of an attack: one affects how quickly you attack, while the other affects the chance of a stronger hit.
Focus Crystal increases damage against enemies within thirteen meters. Its range requirement is the important part of the decision. A Survivor who can safely remain close can make frequent use of it; a run built around shooting from farther away cannot assume that the bonus applies to every target.
AtG Missile Mk. 1 has a 10% chance to fire a missile for 300% total damage, with additional stacks increasing the damage rather than that listed chance. Ukulele has a 25% chance to create chain lightning, with stacks increasing the number of targets and reach. Read what the stack changes before comparing another copy with a new type of item.
Avoid building around an old claim of an infinite Runic Lens and AtG Missile interaction. The October 8, 2026 update fixes a potentially infinite proc chain between them. Use their supported ordinary effects when evaluating the current inventory.
Give group damage an opportunity to trigger
Gasoline activates when you kill an enemy. It ignites nearby enemies, and additional stacks change its reach and burn damage. This makes the first kill in a group important: the effect does not replace your need to finish a target and start the sequence.
Ukulele instead has an on-hit chance to send lightning across targets. That can help spread damage before the first enemy dies. The two effects may support the same broad job, but their triggers differ, so the first few seconds of a fight can feel different depending on which one you have.
Tri-Tip Dagger gives a chance to inflict bleed. Its value comes from landing attacks that can apply the effect and letting the resulting damage contribute to the fight. It is another reason to inspect the attack pattern you actually use rather than compare item names without their triggers.
A build that clears groups comfortably may still need help against a durable single target. Adding more group-only utility is not automatically the answer to a boss that survives your normal attacks. Decide whether the next item is fixing a weakness or merely adding more of a strength you already have.
Match recovery to the available window
Cautious Slug improves base health regeneration while outside combat. Medkit heals two seconds after you take damage, combining a flat amount with a percentage of maximum health. Bustling Fungus creates a healing zone after you stand still for one second. These are different conditions, not three versions of unconditional healing.

If you must keep moving continuously, a stationary healing zone is harder to use. If you can disengage safely, outside-combat regeneration has a clearer opportunity. A delayed heal still requires you to survive until it arrives. Choose a recovery effect that fits the space and timing your Survivor can create.
Acolyte is a special case because her passive redirects ordinary healing into a different exchange. Do not assume that an item restores her actual health exactly as it would restore another Survivor's health. Read the Survivor's own rules alongside the item description when a familiar pickup seems to behave differently.
Damage prevention also differs from healing. Repulsion Armor Plate reduces incoming damage by a flat amount, while Tougher Times supplies a chance to block. Preventing damage can help you reach a recovery window, but it is not the same as restoring health after the hit.
Read stacking according to the effect
Linear stacking means that a specified part of the effect grows by an additional amount per stack. Soldier's Syringe is a straightforward example with an added attack-speed value. A chance-based item may instead keep the same trigger chance while increasing the result's damage, reach, or target count.
Tougher Times uses hyperbolic stacking and is unaffected by luck. Do not turn its displayed per-stack wording into a claim that enough copies guarantee every incoming attack will be blocked. Likewise, an exponential tradeoff should not be treated as a simple fixed subtraction repeated without regard to the underlying rule.
Shaped Glass combines a damage increase with a major maximum-health reduction. Gesture of the Drowned changes equipment cooldown and forces automatic activation when ready. Purity lowers skill cooldowns while making random effects roll for an unfavorable outcome. Each gives a benefit coupled to a change that can undermine another part of the build.
Before seeking another stack, identify what improves and what remains unchanged. Then compare the improvement with a different pickup that covers a missing function. Another damage stack cannot supply an absent jump, and another recovery item cannot automatically fix an equipment timing problem.
Inspect Void corruption before committing
A Void item replaces the stacks of its uncorrupted counterpart with the Void version. It also corrupts future pickups of that counterpart while you hold it. This is an inventory conversion, so collecting the Void item is not simply adding a bonus alongside the original effect.
For example, taking Needletick while holding three Tri-Tip Daggers produces four Needleticks and removes those Daggers. The converted effect uses collapse instead of the original bleed. Decide whether you want that replacement before picking it up, especially when other items or your combat plan depend on the original effect.
The same caution applies to other paired items. Plasma Shrimp replaces AtG Missile Mk. 1, Polylute replaces Ukulele, and Safer Spaces replaces Tougher Times. Their own conditions and results matter more than the fact that they share an acquisition relationship with a familiar item.
Distinguish permanent, temporary and ethereal copies
Temporary items stack with their permanent counterparts, but each temporary copy has a limited lifetime. The standard duration is eighty seconds, extended by Substandard Duplicator. Its timer starts when you pick it up, so a temporary stack is useful for a current encounter rather than a guaranteed part of the entire run.
Temporary items cannot be scrapped or traded through the usual printer, pool, and cauldron exchanges. Do not plan an inventory conversion around copies that those interactions cannot accept.
Ethereal items from Hallowed Concepts last until the end of the current stage. Completing a trial lets you make one collected ethereal item permanent; the others disappear when you move to a new stage. Check which effects remain after travel before treating the previous arena's damage or defense as your permanent baseline.
Hallowed Items, Hexes, and N'tormat pages also have distinct purposes. Hallowed Items offer benefits through special reward routes, Hexes impose penalties for later exchange, and pages unlock rituals. None should be treated as an ordinary chest drop solely because its icon appears in the same broad inventory display.
Use inventory exchanges to preserve the function you need
Scrap has no direct combat benefit and is prioritized by matching 3D Printers. Turning an unwanted item into scrap can protect more useful items from an exchange, but removing the item also removes the effect you were receiving from it. Decide which function you can afford to lose first.
Lunar, Void, Hallowed, and Hex categories do not all follow the normal scrapping rules. Check the category before expecting a Scrapper or printer to accept an object. A visible inventory item is not automatically a valid input for every exchange.
At the end of a selection, check the new inventory against the next encounter: movement, reliable damage, group control, recovery, and equipment use. That functional check is more useful than counting how many rare icons you have collected, and it keeps item choices tied to actions your Survivor can actually perform.
